Policy is not the same as governance
Policy tells people the rules. Governance decides how work actually moves. A business can publish an AI policy and still have no intake process, no approval path, no accountability model, no escalation design, and no clarity on when human review is required.
That is why governance must be operational. It needs to classify use cases, define who can approve what, and set proportional control depth based on the real risk of the workflow.
What bad governance looks like

What good governance looks like
Good AI governance behaves like a delivery system. It defines fast lanes for lower-risk work, deeper review for higher-risk work, and clear handoffs between business owners, product owners, security, privacy, legal, architecture, and support.
The key is proportionality. A drafting assistant used by staff does not need the same control depth as an agent that can retrieve personal information, call external tools, or write into operational systems.
Why this matters more as agents spread
As agentic AI becomes more common, the governance problem gets sharper. Gartner now predicts that by 2028 the average Fortune 500 enterprise could have more than 150,000 agents in use, while only 13% of organisations believe they currently have the right AI agent governance in place.
That gap matters because the next risk is not only bad answers. It is unsafe action: the wrong tool call, the wrong disclosure, the wrong update, or the wrong autonomous step in a workflow.
